Virtual Check-in / Check-out
We are now using a virtual check-in and check-out process. No more clipboards / forms to fill out in the office. You will be emailed or sent a text with a link to fill out your intake info before you arrive. This will allow for a completely contact-less experience. No more pen and paper!
Please be on time
In order to maintain social distancing, spacing between appointments is very important. Late arrivals may be asked to reschedule.
Intake Forms
If you have not completed your intake forms, upon arrival we will request that you complete them. Our staff can email or text the intake forms right to your phone.
Face Masks Required for Entry
For the entirety of your visit, we will be requiring you to wear a face mask covering your nose and mouth. Our staff will do the same.
Only Bring Yourself
Patients under 18 years old must be accompanied by ONE parent or guardian. Patients that need assistance with mobility or communication may also be accompanied by ONE companion. If you MUST bring anyone inside, they will be COVID screened and please be sure they are also wearing a face mask covering their nose and mouth and that they keep more than 6 feet from anyone else.
